SUCCESS FROM THE START

The formation of FBANA in 1995 marked the beginning of a tradition that has grown and evolved over the years. Starting with just five Toronto-cities and basketball leaders Toronto-Ed Semira, Chicago- Ben Libao, Mississauga- Geopo Dacanay, Windsor- Ed Parungao, Vaughan- Zomy Ajon, it quickly expanded to 15 cities, showcasing the growing interest and participation in the annual Labour Day tournament.

FIRST MEDALS

In April 2005, FBANA hosted one of the largest basketball camp ever to be held at the local school or college level.

We've operated within the City Basketball Club and therefore all players had the opportunity to progress through a seamless pathway from basketball lessons to professional players. We also had squads for those who choose to continue basketball for enjoyment.

CHAMPIONS

The basketball programme consists of one 30 min. session per week and all children are grouped on best ability.

FBANA WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in FBANA’s International Basketball Tournament, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS FBANA, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

5. I consent to the use of my image(s) or likeness of my image(s) and am aware that I may be recorded in one of the following formats: film, video, digital photo and/or print. I consent to this recording and am aware that my image may be distributed locally and/or internationally upon completion. I understand the producers have a duty and responsibility to uphold a positive image of FBANA and its affiliates, associates and internal partners. I hereby give and grant FBANA, its clients, agents and vendors the absolute and unconditional right to use, publish, display, and/or reproduce in any manner, video/photographs in connection with educational, recruitment, admission or trade materials.

6. I hereby waive any right to inspect and approve the finished video/photographers or the manner in which any of the same are used, reproduced, published or displayed.

7. I further release FBANA, its clients, agent or vendors from any liability whatsoever that may occur or be produced in the taking, reproducing, publishing, showing, or displaying of said video/photographs, and agree that FBANA Inc. shall be the owner/photographs, all rights to them, may copyright the video/photographs in its own name, and may grant to others permission to use them.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
